Dear whoever fills out the forms,

If you don't put a patient's name on the form, and you don't put a doctor or hospital's name on the form, the whole thing is pretty much useless. We can't even send it back to you to have you try again.

Now according to DD Form 567, if you don't know the name of the soldier you may put unidentified and send it through the process and let the guys doing the DNA figure it out. Of course that's just me and what works with my training :)

If they can't manage to get the name on the paperwork, I don't think I'd trust any DNA sample they would send us.
